INFORMATION ABOUT THE INSURANCE BROKER

in accordance with Act No. 170/2018 Coll., on the distribution of insurance and reinsurance, as amended

1. Rhapsody Insurance s.r.o., with its registered office at Prague 4, Nusle, Lomnického 1705/9, Post Code 140 00, Company ID: 27256961

a/ is a legal entity, which, as of January 31, 2006, based on the Decision of the Ministry of Finance of the Czech Republic, the Office of State Supervision in the Insurance Industry and Pension Supplemental Insurance, is duly registered in the register of insurance intermediaries and independent claim adjusters maintained by the Czech National Bank as the current competent supervisory authority in position Insurance Broker within the meaning of the provisions of § 6 to § 14 of Act No. 170/2018 Coll., on the distribution of insurance and reinsurance, as amended (hereinafter referred to as the "Insurance Broker"),

b/ is a business entity conducting its activities in accordance with applicable laws and other generally binding legal regulations. It holds all necessary permits, licenses, statements, consents, and other decisions required for conducting its business activities, i.e., providing services in selected sectors of non-life insurance,

c/ has valid and effective professional liability insurance in accordance with applicable legislation, which it maintains on a permanent basis so that its parameters always correspond to the current legal standard.

2. In accordance with the relevant provisions of Act No. 170/2018 Coll., on the distribution of insurance and reinsurance, as amended, particularly the obligations set out in Section 87 of the aforementioned Act, the Insurance Broker informs that:

• it performs insurance intermediation activities as an Independent Intermediary, in the position of Insurance Broker as defined in Section 12, Paragraph 1, Point b) of Act No. 170/2018 Coll., on the distribution of insurance and reinsurance, as amended, or in the position of Insurance Agent as defined in Section 12, Paragraph 1, Point a) of Act No. 170/2018 Coll., on the distribution of insurance and reinsurance, as amended,
• the registration of the Insurance Broker can be verified in the register of insurance intermediaries and independent claims adjusters maintained by the Czech National Bank on the website www.cnb.cz or via postal address Česká národní banka, Na Příkopě 28, Prague 1, Postal Code 115 03,
• it is not authorized to receive insurance premiums,
• it is authorized to provide its services in the Czech Republic, Hungary, Germany, Poland, Austria, and Slovakia,

• it does not hold any direct or indirect shares in the voting rights or the share capital of any of the insurance companies,
• neither any insurance company, nor any person controlling such an insurance company, holds any direct or indirect shares in the voting rights or the share capital of the Insurance Broker's company,
• any complaint from an insurance applicant / client can be submitted in writing to the correspondence address listed on the Insurance Broker's website or to its email address info@rhapsodyinsurance.cz or delivered personally by prior appointment.
